Coach Evaluation Form

To all parents:

Your child’s participation in the St. Albert the Great Athletic Program is intended to be a positive and beneficial experience. More often than not, the quality of this experience depends on the relationship between the player and the coaching staff. This questionnaire is one of the tools used to improve the performance of our coaches. Please sit down with your child and provide a thoughtful and honest evaluation of your team’s coaches. You are not required to use your name, but you may do so if you wish.

Thank you!
